Carhart’s notch was seen at 2 kHz in 28 (93 per cent) patients but at 1 kHz in only two (7 per cent). However, in patients with chronic otitis media, a Carhart’s notch was seen at 1 kHz in 10 (55 per cent) patients and at 2 kHz in eight (45 per cent) patients.

Introduction: Carhart's notch (CN) is a false depression of bone conduction (BC) thresholds at 2-4 kHz initially described in cases of stapes fixation. This study was designed to estimate the incidence and assess the clinical significance of CN in cases of otitis media with effusion (OME) in children. Carhart notch, described for the first time in 1950, involves a reduction in the bone curve by approximately 10 to 20 dB at a frequency of 2000 Hz. It arises as a result of mechanical immobilization of the stirrup base in the oval window and disturbances in the self-resonance of the ossicular chain. The Carhart notch is not a true indication of "cochlear reserve" and this apparent bone conduction loss may be corrected by surgical intervention (Tonndorf, 1971). Carhart (1971) and Tonndorf (1971) provided a review of several theories to explain this phenomenon. For Carhart notch to be used as a preoperative predictor of stapes fixation, it should be shown that the notch exists with stapes fixation but not with other ossicular chain disorders, such as disconnection; however, few clinical investigations have assessed this issue.

2019-09-26 · Carhart Notch-A Prognostic Factor in Surgery for Otosclerosis.

The Carhart notch is an elevation in the middle-frequency bone-conduction threshold of an ear with clinical otosclerosis. The study population consisted of 138 patients with clinical otosclerosis. Conventional air-conduction (AC) and bone-conduction (BC) and electric bone-conduction (EBC, with Audimax 500 audiometer) thresholds were measured 2 weeks before and 7 months after stapes surgery

Objective: To evaluate the ability of the Carhart notch to predict the presence of effusion at the time of myringotomy and to evaluate its incidence in otitis media with effusion. Method: A prospective study that comprised 75 children who were admitted to King Fahd Hospital of the University (KFHU), Al Khobar, Saudi Arabia for myringotomy and possible ventilation tube insertion as treatment 2021-02-24 Audiologist Raymond T. Carhart in 1950 was first to show in these patients a 10–20 dB BC hearing loss in the narrow 1–2 kHz range of input frequencies, with no significant hearing loss at lower and higher frequencies, which has since been named ‘Carhart’s notch’ (CN).

This produces a characteristic loss in bone-conducted (BC) hearing of about 20 dB between 1 and 2 kHz, known as "Carhart's notch", for which the specific mechanisms responsible have not yet been well understood.

A deficiency particularly around 2 kHz (termed a Carhart notch in the audiogram) is characteristic of either otosclerosis or a congenital ossicular anomaly.. talar starkt för diagnosen; [Audiogram med ”Carharts notch” vid 1,5-2 kHz, d.v.s. benledningen tycks försämrad specifikt vid dessa frekvenser]

The study population consisted of 138 patients with clinical otosclerosis. Conventional air-conduction (AC) and bone-conduction (BC) and electric bone-conduction (EBC, with Audimax 500 audiometer) thresholds were measured 2 weeks before and 7 months after stapes surgery A Carhart's notch at 2 kHz indicates stapes footplate fixation, whereas one at 1 kHz indicates a mobile stapes footplate; the footplate mobility can thus be predicted pre-operatively.

This retrospective audiometric database and chart review in a tertiary referral centre was performed with What is the Carhart notch in Pure Tone Audiometry? Stimulation of the ear via bone conducted vibrations is a well-established way to differentiate conductive from sensorineural hearing loss – but paradoxically, the accurate testing of bone conduction thresholds depends on normal middle ear function. 2015-10-19 #carhart #notch #otospongiosisDip at 2000Hz Bone Conduction frequencyPlaylist https://www.youtube.com/playlist?list=PLKKWBex6QaMCojCF2tDToiD-nNhVZbIyO Otosclerosis is a condition of the inner ear where one or more foci of irregularly laid spongy bone replace part of normally dense enchondral layer of bony otic capsule in the bony labyrinth. This condition affects one of the ossicles (the stapes) resulting in hearing loss, tinnitus, vertigo or a combination of symptoms. The term otosclerosis is something of a misnomer. A study that considered the Carhart notch as one of the factors affecting the postoperative hearing has concluded that stapedotomy is the procedure with the highest efficiency in the treatment of 2020-05-12 Objective: The hallmark of bone conduction audiometry in otosclerosis is the "Carhart notch."The mechanism for this phenomenon is still not clearly understood. The objective of this study was to describe the pattern of bone-conduction threshold responses preoperatively in patients with otosclerosis and to assess overclosure after laser-assisted stapedotomy.
